**IMPORTANT** If you have not completed the online PCF form in Skyward Family, please do so as soon as possible.
Pursuant to Florida Statute 1001.42, HB 1557: A parent/guardian MUST Opt In for Health services yearly. Opt in MUST be completed for screenings, clinic services, and other health services. Complete OPT in section on the 23-24 Emergency Information Form to be sent home first couple weeks of school, or directions to complete digital document:

Chronic Medical Conditions
Chronic medical conditions should be discussed at the beginning of the school year with the school nurse. Parents are required to complete the emergency information form each year and provide current working phone numbers of all contacts listed. All in-school procedures (catheterizations, feeding tubes, special diets, nebulizer treatments, glucose check, etc.) require documentation by your student’s physician. These forms require physician's signature and are available in the clinic and must be renewed every school year.
Diabetic student needs a current DMMP from their physician each school year. Student with seizure needs ISP (individual seizure plan) which is provided by the student physician.

For safety and security reasons, students are not permitted to bring medications to school (exception: Tylenol/ Ibuprofen, epi pens, rescue inhales, diabetic supplies, pancreatic enzymes) Whenever possible, medication should be taken at home. If your student must take medication at school, the following OCPS policy will be followed:
1. All medication must be administered by the school nurse or designated trained person.
2. The parent/guardian must bring the medication to school and complete the authorization form. Authorization forms will be signed upon receipt and must be completed every school year.
3. Prescription medications must be in the original pharmacy labeled container. Parents can ask their pharmacist to provide them with two containers: one for school and one for home. Medications including eye drops cannot be transferred to and from school.

Inhalers, Epi-pens, diabetic supplies, and pancreatic enzymes may be carried by the student, provided the proper authorization forms are on file with the school. These forms require physician signature and are available in the clinic and must be renewed every school year. A backup in the clinic is always advisable. These forms must be signed by a physician that is not the parent.

Effective 2023-24 school year the clinic will no longer stock/provide OTC medications (such as ibuprofen (generic Advil), acetaminophen (generic Tylenol), calcium carbonate (generic Tums). Per OCPS, based on Florida House Bill 1537,
students may carry ibuprofen and acetaminophen, however if your student is a frequent user it is advisable to have a backup supply in the clinic for those days when they forget. Medications must be provided by a parent/guardian in an unopened container with a signed medication authorization form. Authorization forms will be signed upon receipt and must be completed every school year.

Physician Forms
Self-carry forms for asthma and epinephrine may be accessed at
https://www.ocps.net/departments/health_services. These forms can only be signed by a physician that isn't the student's parent. Once the form is complete, please bring it to the school clinic, along with the child's unopened medication. Please make sure the pharmacy label remains on the container and verify the expiration date has not passed.

Injuries
Please provide the school nurse with medical documentation of injury and restrictions/accommodations ordered by the physician. If a student requires extra time between classes and/or elevator use a pass will be provided. Medical documentation is required to be excused from PE.

Illness
Please keep your student home if they have a fever of 100.5 or higher until they are fever-free without the use of fever-reducing medications for 24 hours. Send in an excuse note with the student within 2 days of absence.
